When this form is first opened using the link from the Academy’s website, only part one will be visible, where the form user provides some basic information about the application before selecting the green “start application” button at the bottom of the page. This ensures a record of the application is created immediately. The next steps are outlined below:
1) Once “start application” is selected, a link to the Academy’s MTI data protection consent (DPC) form is sent to the email address entered in the “Email address of individual requesting form” field, as well as the royal college (if any). This link must be forwarded to the doctor so they can complete the DPC form.
2) After the doctor has completed the DPC form, a link to their application will be sent to the email address entered in the “Email address of individual requesting form” field, as well as the royal college (if any).
3) The employer and royal college (if any) will then complete parts 1-7 of the application. More information is available below about the deanery approval process and providing evidence of GMC registration and licence to practise in the UK, as there is some flexibility with these items.
4) Once parts 1-7 have been completed and the green “continue to sign” button is selected in part seven, the doctor’s medical personnel will be contacted at the email address listed for them in part seven to sign the application.
5) After the medical personnel has signed, the supervising consultant will be contacted at the email address listed for them in part seven.
6) The doctor will then be contacted with a private link to upload their passport or biometric residence permit (whichever applies) to the Academy. This adds an additional layer of security to the process as the link is sent to the doctor only.
7) The employer and royal college (if any) will be notified once the doctor has uploaded their passport or BRP.
8) If no further information is required, the application will go to the employer or royal college (if any) for a final review.
9) Once reviewed, the application will go to the Academy for review.
Deanery approval:
We recommend that deanery approval is obtained using our form before beginning this application form.
A separate form must be submitted to the regional postgraduate deanery to approve the details of an MTI post. This applies to all extensions and transfers as well. In part four of this application form, you will be asked if the Dean has approved the post already, and if not, whether an approval request has been submitted to them. If “no” is selected to both questions, the next time the form is saved a link to the deanery approval form will be provided in the following email auto-response.
Most of the fields in the form will be pre-filled using the information added to part three of this application form. Some additional information also needs to be added for the Dean to properly assess the hospital’s training capacity. You will also need to add the MTI application ID, which will be provided in the above-mentioned email auto-response. This will help link the two forms together.
Once the Dean has approved the post, a PDF record of their approval will be provided to all email addresses listed on this application form. This PDF record will need to be added to part four.
GMC evidence:
For extension or transfer applications, evidence of GMC registration and licence to practise must be uploaded to the application before it can be sent to the medical personnel and supervising consultant to be signed. This also applies for new placement applications if the doctor is already registered with the GMC.
For new placement applications for doctors who are not already registered with the GMC and are being sponsored by their employer or royal college, the employer or royal college can add evidence of GMC registration and licence to practise at a later stage in the process, if necessary.
After noting in part three that the doctor is being sponsored by a royal college or their employer, the field “Evidence of GMC registration and licence to practise can be provided now or at a later point if it is not yet available. Please select one of the options below:” will appear. Simply select “I would like to upload evidence at a later point”, and you will be allowed to proceed to the signature stage without uploading evidence. A reminder email will be sent to all email addresses listed on the form to upload this evidence before the application is sent for final review.
