Thank you for your interest in getting tattooed! This form is made to help me streamline my booking process and cut down on the back and forth that can come with scheduling a tattoo.

After filling out this form I will reach out via email to confirm an available date and finish the booking process.

I take a $75 deposit for all tattoos so please be deposit ready at the time of filling out this form.
*note: all deposits are non-refundable*

Are you looking to get a tattoo from my original flash?

Please give a detailed description of the tattoo you would like to get. Include size, placement, style, color, and any other information you think might be helpful for the drawing process.

Please include any inspiration/reference photos for the tattoo. As well as, a well lit picture of the area you would like tattooed if it is near any other tattoos or is a cover up.
