International Travel Health Certificates require time and research. Every country has different requirements for entry and it is on the Pet Parent to do their due diligence in regards to the Country of Destination requirements and USDA procedures.

- Some Countries require an embossed seal with original USDA Veterinarian Endorsement signature instead of an electronic signature. If this is stated in the Country of Destination's requirements then review the links below for how to proceed.

Depending on the Destination Country, repeat vaccines, titers, microchips, specialized deworming etc. may be required and we want to ensure your pet has everything they need to travel with you PRIOR to your scheduled Health Certificate appointment.
This way, if additional steps are needed we have time to collaborate and schedule them accordingly before your departure.
- Failure to do so will result in your pet being denied entry to the Destination Country or your pet being held in quarantine at your additional expense.
This form must be completed in its entirety in order for us to complete our portion of your pets travel documents.
If traveling with more then one pet you will need to complete additional forms for each pet.
It is recommended that you call or visit your Airlines website and determine any specific travel requirements your Airline requires, as each Airline is different and some have additional forms that need to be filled out by your Veterinarian, which we will need ahead of your appointment.
