            Now that you have enrolled, you can schedule your first visit and will be contacted via email to complete the appropriate paperwork for your enrollment. 

            As a member, you will receive 15% off of Lab testing and supplements. 

            **If you desire to cancel, it will require 30 day written notice**

            **Should you need to cancel prior to your first appointment - the $50 enrollment fee with be retained - non-refundable**
