    Thank you for choosing to book your next tattoo with me! In order to help me streamline the booking process please fill out any relevant information I may need for your appointment below. If there is a question you do not understand or are not sure how to answer, give a best guess and let me know at the end how I can help. Once this has been filled out you will be automatically put onto a waitlist and from then you will be contacted by my assistant, Nathan, or I for more information. Please allow me at least a week or so to respond. If it is an emergency please message me on my Instagram @foxs_tattoos. All other matters will be handled through email. Note that flashes have priority in my booking over custom designs. I do not hand out custom designs ahead of time to view because often I draw the design the night before or the day of. The design will be shown to you when you come to your appointment. Minor adjustments can be made day of your appointment and if they are major changes I may have to reschedule you for a later date to allow me time to make those changes. If you are booking for a place I am guest spotting at please let me know in the additional comments section at the end of the form. Thank you!

    Once I reviewed the details of your tattoo and we have confirmed a day via email a $100 deposit will be required to secure your spot in my schedule. Deposits will go toward the full cost of your tattoo at the time of your appointment and are non-refundable. If you need to reschedule please let me know at least a week ahead of time and an additional $50 deposit will have to be immediately paid that day. If for any reason, you need to reschedule a second time, a $50 fee will be required to pick a new date and this will no longer go towards the full cost of your tattoo. All deposits will be sent either to my Zelle, Venmo or CashApp. No other payment methods will be accepted for deposits. I am no longer holding a deposit for you longer than a month if you cannot choose a rescheduled time for your appointment. A new deposit will have to be put down beyond that.
