You need to complete this form if you are requesting an advertised player base fee reduction for reasons other than a family financial situation. If you feel there is a need for a reduction due to a family's financial situation then you will need to have the family complete a hardship request. The request will be presented during the first board meeting after receipt of the request. The coach submitting the request is encouraged to be present at the board meeting to answer additional questions if needed.
