By completing this form, I confirm that the information that I have provided is accurate and true. I acknowledge that this application is an inquiry only and that an email confirmation with the details of my order will still be sent to me through my contact method indicated above. I accept that the email confirmation will be based on the selections that I have made and any changes required will be communicated to the Catering Team at BAO MAMA via email at catering@baomama.ca. I accept paying a deposit that is 50% of my order total, via etransfer to catering@baomama.ca as confirmation to proceed with my order. I acknowledge that all catering requests require a minimum 1 week notice and that requests made less than 48 hours may be subject to an 18% expedite fee. I accept that any special handling requests are subject to an addional service fee. If applicable, I accept a delivery fee of $0.85/km to and from the restaurant to my requested location. I consent to the use of my telephone number and/or e-mail address to be contacted for the purpose of this and any orders that I may place in the future.

I acknowledge and accept that BAO MAMA's food products and/or ingredients may contain or have been in contact with dairy, egg, wheat, peanuts, tree-nuts, soy, fish, shell fish.
