Course Certificate Re-Issue Request
After you fulfill all course requirements complete the evaluation survey, and pass the final exam or assessment activity, you will earn a Certificate of Completion in the Learning Center. It is your responsibility to download the certificate for your own records within 6 months after the course occurs. If you lost your certificate, please complete this Certificate Request Form to get a certificate re-issued within the last 5 years.

Certificates older than 5 years will not be re-issued.

For certificates from within the last 5 years, please submit your request below so that we can verify and re-issue your certificate. Once you submit and we verify your completion status, you will then be emailed a signed Certificate of Completion from hello@aptapelvichealth.org. Please fully and accurately complete all the fields below (including the correct format for dates and location) for EACH individual certificate you are requesting. Failure to do so may result in your request being denied.
