2024-2025 High School Confirmation Sponsor Form

I have been asked to be a godparent/sponsor for the below listed person as he/she complete the Sacraments of Initiation in the Roman Catholic Church.

Canon Law 874:

To be admitted to the role of godparent/sponsor, a person must be a Catholic who has been Confirmed in the Catholic Church, has already received the Sacrament of Holy Communion and leads a life in harmony with the faith and the role to be undertaken.

In accepting this responsibility, I affirm that:*

I have received the three Sacraments of Initiation into the Catholic Church (Baptism, Holy Eucharist and Confirmation).I will be at least 16 years of age by the Confirmation.I am a practicing Catholic: I participate in weekly Sunday Mass and the holy days of obligations, and I give witness to my faith in Jesus Christ by the worthy reception of Holy Communion.I am EITHER not married, OR, if married, I am married according to the laws of the Catholic Church (i.e. not married outside the Church without a dispensation, and not remarried without an annulment).I am not bound by any canonical penalty legitimately imposed or declared.I am aware that I am assuming the responsibility to be a good role model for the person I am sponsoring by my life of prayer and by my Christian example in order to help the candidate to act as a true witness to Christ and to faith full fulfill the obligations connected with is Sacrament.I am not the parent or spouse of the Catechumen/Candidate.I understand that all of the above boxes need to be checked in order to be a godparent/sponsor.If not all boxes are checked I will let the person, who is asking me to be their sponsor, know that I do not have all of the qualifications.
