  • Pet Information

  • If necessary, will someone in your home be able to safely restrain your pet for a brief sedation injection?
  • Do you have pet insurance? Euthanasia may be covered under your policy.
  • How did you hear about Tender Transitions? You may chose more than one.

  • Aftercare Options:

  • It is my desire to provide humane aftercare for my deceased pet that complies with all state, provincial, and local laws. I further authorize the attending veterinarian to have said animal's body cremated in accordance with the above policies via the option I have selected below, and I accept responsibility for the expense incurred.*
  • The private cremation option includes having your pet's ashes returned in a beautifully carved wooden urn with custom brass plate name engraving or having your pet's ashes returned in a blue scatter tube for easy dispersal of ashes. Return of ashes options:
  • For cremation services:

Small pets will be gently placed in a basket for transportation after the appointment concludes. Larger pets will be gently wrapped in a blanket and placed on a stretcher for transportation. If a stretcher is needed due to the pet's size or weight, an additional able-bodied person will need to be present at the appointment (either owner, friend, neighbor, etc plus the veterinarian) to help transport your pet to the veterinarian's vehicle for aftercare. For pets over 80 lbs, there will need to be two able-bodied persons (plus the veterinarian) provided by the client to help carry the stretcher. If you do not have anyone who can be present at the appointment to help transport, alternate arrangements for transport from your home to the veterinarian's vehicle can be made for an additional fee.

  • I understand additional help may be needed to transport my pet to the veterinarian's vehicle after the appointment based on my pet's weight and size. I am capable of carrying up to 40 lbs to assist my pet out of my home on a stretcher, or I will provide someone who can help instead. Likewise, if my pet is over 80 lbs, I will have two able-bodied persons who can each carry up to 40 lbs each to assist with transport. If other assistance is required, I will inform the veterinarian so other arrangements can be made to help transport my pet for cremation after the appointment.
