  • By submitting this Facilities Request Application, you represent that you have the authority to act on behalf of your organization. If your application is initially approved, a representative will contact you to enter into a Facilities Licensing Agreement. Only after you enter into a license agreement and provide insurance certificates that are satisfactory to the representative will you or your organization be permitted to use the facilities as requested.
