VII. Acknowledgements and Agreements

I understand I need to provide a $25.00 non-refundable application fee, payable to Arizona Foundation for Women in order for this application to be considered for admission.

If selected, I understand a non-refundable tuition fee of $400.00, payable to Arizona Foundation for Women is due upon acceptance and must be paid by October 13th, 2023. Due to the demand for the program, I understand that my seat may be forfeited and given to another candidate if the tuition fee is not paid by this deadline.

Attendance: You and your fellow program participants all benefit from your attendance, therefore, 100% attendance is the preferred for all participants. If selected, I will attend the orientation, fundraising workshop, pairing event and 5 of the 6curriculum sessions. Full participation is defined as arriving on time, staying until each program session is adjourned and, with few exceptions, not using your cell phone or transacting business once the program session has begun. The emphasis, even during breaks, should be on interacting with your classmates and presenters. If there is an emergency or unforeseen circumstance, please contact the Program Director, Tricia Livermore at tricia@azfw.org, as soon as an absence is anticipated.

I have reviewed the requirements of the SHE Leads! program and understand I need to achieve the following in order to graduate - raise the $2,000 fundraising requirement (or $500 if company pays the $2,000), attend 5 of the 6 curriculum sessions and meet with my Mentor (in person or virtually) for at least one hour every month from January - June 2024.
