Waiver and Release Form
In return for being allowed to participate in Chair-ity volunteer activities and all related activities, including any incidental activities ("Volunteer Activities"), the undersigned Volunteer (or Parent/Legal Guardian if under 18) agrees as follows:
Release of Liability
I, the undersigned, hereby waive and release and agree not to sue Chair-ity or its officers, directors, employees, sub-contractors, sponsors, agents, affiliates (“Chair-ity”) from any and all present and future claims that may be made by me, my family, estate, heirs, or assigns for property damage, personal injury, or wrongful death resulting from participation in the Volunteer Activities, whether arising out of ordinary negligence or otherwise.

I understand that:
- Chair-ity is not responsible for any injury or property damage resulting from the Volunteer Activities.
- The Volunteer Activities may involve inherent risks, including serious injury or death.
- I am voluntarily participating and accept all such risks.
- I will indemnify and hold harmless Chair-ity from all claims arising from participation.
- Chair-ity carries no insurance for my benefit.
- I am in good health and physically capable of participating.
- This document is a binding legal contract waiving certain rights and eliminating Chair-ity’s liability.
Publicity Release
I also grant Chair-ity and its authorized parties the irrevocable right to use my name, voice, photograph, and likeness in current or modified form in any promotional, advertising, or other media-related content without additional compensation. This includes all forms of media now known or later developed, and I waive the right to approve or be compensated for such use. I understand I may not authorize use of these materials by others.

If any provision of this agreement is deemed invalid, the remainder shall continue in full legal force and effect.
