I,{name}, hereby consent and authorize Dot Lake Village, its agencies, legal representatives, successors, and assigns, to use and reproduce these photographs, still or moving, by or for [Institution name], alone or in conjunction with other photographs, still or moving, sketches, cartoons, artwork and text matter, and circulate the same for the purposes of illustration, advertising and publication in any manner and in any medium whatsoever without limitation or reservation (including but not limited to use in books, magazines, periodicals, newspapers, brochures, pamphlets, leaflets, all types of printed material, web pages, and in television, and all other means of public display).

 

I specifically release [Institution name], its agencies, legal representatives, successors, and assigns, of any and all liability of any and every nature arising out of the above-mentioned use of these photographs on behalf of myself, my agents, legal representatives, successors, and assigns.

 

I understand that use of photographs will be limited to educational, non-commercial purposes.  No other oral or written representations or promises have been made to me.

Liability Waiver

 

I acknowledge and fully understand that Boating activities inherently involve risk of damage to property, serious injury and even death, whether as a result of my own choices and actions or of the actions, inaction and/or negligence of others. Nevertheless, despite these inherent risks, I choose to continue to participate in Boating activities. I alone shall be responsible for the use of my own judgment in determining whether I should participate in any Dot Lake Village Council sponsored activities. Further I, for myself, my heirs, executors, administrators and assigns, and any other members of my household, forever waive, release and discharge any and all rights, demands, and claims for damages I may have against Dot Lake Village Council, their representatives, or outing leaders for any and all loss or injuries in any manner arising out of or resulting from my participation in any of the activities of Dot Lake Village Council.

I freely and voluntarily assume the responsibility for 1) proper selection, maintenance, and use of equipment; 2) assessing water conditions and level of difficulty; 3) judging the level of risk and whether my skill level is adequate to safely meet the risks; and 4) my physical condition and conditioning. I also freely and voluntarily assume all risks as incident to participation, including but not limited to 1) cold, heat, and risks of hypothermia, hyperthermia, sunburn, and dehydration; 2) striking or being struck by other paddlers, paddling and game equipment, boats, and other objects; 3) the risk of being caught in hydraulics; and 4) not being rescued before injury or death occurs. I will assume and pay my own medical and emergency expenses in the event of an accident, illness or other incapacity regardless of whether I have authorized such expenses.

I will use a properly fitted Personal Flotation Device (Life Jacket) and other required equipment when participating in a Dot Lake Village Council event. I will behave in an appropriate manner to avoid injury to myself or others.

I will not use Dot Lake Village Council equipment or facilities while I am ill with an illness that is transmittable through normal contact and use of the equipment and facilities.
