WSA Soccer utilizes exclusively Direct Deposit via a payroll provider, PayCor.

The information collected on this form is required for your pay via PayCor and compliance via IRS regulations for Independent Contractors.

BANKING INFO IS OPTIONAL TO PROVIDE - PLEASE READ!

If you do not provide your banking info, then you will need to log in to PayCor once your PayCor account is set up by WSA Human Resources Department. Then you will add your bank routing and bank account information and be eligible to be paid.

If you do provide the banking info as requested below, we will set up your PayCor Account in full and your direct deposit will begin immediately based on the information you have provided. We then encourage you to log in to PayCor, set up your own password and login credentials, so that you have access to your pay stubs, tax documents for end of year filing, and ensure all account information is accurate such as banking info.
