Veterinary Physiotherapy Treatment Consent
Please read the following carefully before signing.
I confirm that I have provided accurate and complete information regarding my animal’s medical history, current health status, medications, and any veterinary diagnoses. I understand that it is my responsibility to inform the therapist of any changes to my animal’s health before each treatment.
Veterinary physiotherapy, including manual therapy, Class IV laser therapy and Pulsed Electromagnetic Field (PEMF) therapy, is generally considered safe when carried out by a qualified professional. As with any therapeutic treatment, there is a small possibility of temporary or unexpected reactions. These may include mild soreness, fatigue, stiffness, increased sensitivity, temporary worsening of existing symptoms as part of the healing response, skin irritation, or, in rare cases, no improvement in the presenting condition.
Class IV laser therapy and PEMF therapy are not suitable for every animal. Treatment may be contraindicated or require modification in certain circumstances, including but not limited to:
Pregnancy.
Active haemorrhage or significant bleeding disorders.
Known or suspected cancer in or near the treatment area, unless specifically approved by the attending veterinary surgeon.
Areas of active infection where treatment is not clinically indicated.
Over implanted electronic medical devices (where applicable).
Any other condition that, in the professional judgement of the therapist or referring veterinary surgeon, makes treatment unsuitable.
Every effort is made to minimise risk through a thorough assessment and adherence to current professional guidelines. However, no guarantee can be given regarding treatment outcomes, and individual responses to therapy will vary.
While every reasonable care is taken during assessment and treatment, I acknowledge that animals may react unpredictably. I agree that the therapist shall not be held liable for illness, injury, deterioration of an existing condition, or unforeseen adverse reactions that may occur during or following treatment, except where caused by negligence.

I have had the opportunity to ask questions and have received satisfactory answers.
I consent to my animal receiving veterinary physiotherapy, including manual therapy, Class IV laser therapy and/or PEMF therapy where clinically appropriate.
I understand that I may withdraw my consent at any time before or during treatment
