I certify that I am the writer of the song(s) listed below and have registered the song(s) with BMI or other music rights management company. By signing this agreement below, I grant my permission for Finding Faith Radio to play the song(s) on their Live365 Gospel Music Internet Radio station beginning on the date this form is signed. I agree to notify Finding Faith Radio in writing if I should decide I no longer want the songs played.
