Please complete this form to start your registration for the senior trip. Please note that this form alone does NOT reserve your spot on the trip. Upon completion of this form, you will need to provide a $200 non-refundable deposit in the form of cash or a check made out to Magnolia Travel Group to Natalie Adkins to lock in your spot. If you have any questions, call or text Natalie at 601-214-3853. Natalie and Rebecca Butler, our travel advisor, will be at Panera Bread at Renaissance between 5 - 7 p.m., on Wednesday, July 10, 2024, to collect your forms and deposits and answer any trip-related questions.

I acknowledge that completing this form does not complete my registration. I will bring a $200 non-refundable deposit by check or cash to Panera Bread at Renaissance in Ridgeland on Wednesday, July 10, 2024, to lock in my spot. I acknowledge that the deposit is non-refundable.
Airfare & Additional Charges*

I acknowledge there will be additional charges for this trip such as airfare, transfers to and from the airport, and other optional excursions and events. Pricing, booking instructions, and breakdown for each item will be sent at a later date.
Cancellation & Travel Insurance*

I acknowledge that Travel Guard will be used for trip insurance and quotes will be provided per family as requested. I also acknowledge that Travel Guard's Preferred Policy for cancellation is for medical, death, or work-related reasons, or for a positive Covid test, and that the policy also provides medical coverage while on the trip.
Non-Refundable Deposit*

I acknowledge a non-refundable deposit of $200 per room is required, and I plan to provide the deposit along with a printed copy of this form based upon the above instructions. (When you submit the electronic version of this form, you'll receive a PDF to print. If you are using a Comcast email address, you may not receive the confirmation email, but Natalie will have an electronic copy.)
Sibling/Parent Policy*
