  • Arrival:

    Please be on time. After 10 minutes your appointment may be subject to cancellation. If needing to reschedule you may do so via text at least 24 hours prior to your appointment. 

     

    Come to your appointment with squeaky clean lashes. Your lashes and surrounding area should be free of makeup, mascara, lash glue, oils or lotions. Dirty lashes cause poor retention resulting in a rapid loss of lash extensions. If a lash bath is needed it will be an $10 additional charge.

     

    Refills:

    All refills are required within 2-3 weeks of your first appointment, and you should have 40% or more of your extensions to be considered a refill. Any appointment over the 3 week mark is considered a full set.

     

    Cancellations/No Shows:

    Please be on time. After 10 minutes your appointment is subject to cancellation. if needing to reschedule you can do so via text or WhatsApp at least 24 hours prior to your appointment. Like such, a 24 hour notification is required for any cancellation or change in your appointment. Under 25 hours of notification will be charged a cancellation fee of $25. 

     

    Due to the nature of this service, I kindly ask that you please do not bring any additional guests/children.

  • BEFORE THE PROCEDURE

    1. Avoid makeup application and oily products. Face and lashes should be free of makeup, eyeliner, mascara the glue will not adhere to the natural lash and its possible that the undereye tape will not adhere to the skinduring the service.

    2. Do not curl your lashes.

    3. If you use contact lenses please remove prior to your procedure. 

    4. If you have any allergies please disclose information with technician.

    5. Please do not have caffeine prior to appointment, Having caffeine can result in very fluttery eyelids making it difficult to apply the lash extensions.

     

    AFTER THE PROCEDURE

    1. Avoid contact with water 24-48 hours after the procedure.
    2. Avoid extreme heat such as, steam rooms, sauna, jacuzzi, or sweating.
    3. Avoid products with an oily base/finish. Do not curl the lashes extensions. Do not tint the lashes.
    4. Do not use mascara on extensions or lower lashes. Do not apply false lashes as they will not be needed.
    5. Avoid rubbing and picking eye and NEVER ATTEMPT TO REMOVE LASHES if not by a professional. The adhesive is strong and you may damage your natural lashes in removal attempt. If you feel any discomfort during or after the application, please contact us as soon as possible. We will guide you and find a solution.

  • Type of medical conditional that may contribute to the loss of hair and lashes: hyperthyroidism or hypothyroidism, alopecia areata, lupus, diabetes*
  • Medications that may contribute to hair or eyelash loss: Chemotherapeutic agents used in cancer treatment, anticoagulants (blood thinners), beta-blockers (used to control blood pressure)*

    I release Stacey Velez from all responsibility. I understand and have been oriented on the risks of allergies and reactions and that I should see a health professional in case of having an allergic reaction which does not subside in less than 24 hours. By signing this consent, you authorize Stacey Velez to perform the service(s) on me.

  • I agree to carry out the procedure of eyelash extension by a certified professional, Stacey Velez. I agree to use only the products recommended by the salon and Stacey Velez. I understand that there are several factors that influence the duration of eyelid extensions for example: If I take any medication that stimulates hair growth, if I expose my eyelids to hot water steam, sauna, jacuzzi or very hot temperature water. The period of ovulation also influences the duration of eyelid extensions.


    I understand that the service of eyelash extensions is a semi-permanent process. Since the extensions are placed on my natural eyelids and not on my skin, my natural lashes go through a life cycle that lasts approximately 35 to 100 days.
    I understand that a maximum of 7 extensions can be dropped per day due to the cycle of my natural skin.
    I understand that I should not use a mascara on my eyelash extensions as well as the lower lashes either.
    I was notified that I should not use makeup removers that contain oil. I understand that my makeup should be removed with oil-free products.
    I understand that I have to schedule my refills every two to three weeks after my first visit. 40% of the work done in my full set is refilled. In case it is more than 40%, the technician will determine the cost to pay.
    I understand that i cannot rub by eyes or pick at my eyelashes. because this can cause the loss of my eyelash extensions.
    I was notified that I should keep my eyelash extensions clean and that I must wash them with a lash shampoo and not with another product that has not been recommended by Stacey Velez.


    I must immediately notify Stacey Velez if I feel any itching or discomfort.


    I was notified that the work done by the eyelash extension technician has a 48-hour warranty and this covers the loss of excess lash extensions or any changes to the lash extensions.
    I have 5 days to get my warranty. If the 5 days pass, you may not be seen as the guarantee will be invalid.
    I don’t have to find my eyelid extensions as this would cause my natural eyelids to be lost by traction.


    I have to notify the skin extension technician, Stacey Velez, if I am allergic to latex, cyanocrylate, carbon, formaldehyde or any other chemical that is not mentioned here.
    I understand that by performing the procedure and I suffer some sort of allergy I have to notify the technique for the removal of eyelid extensions and that the money will not be refunded.


    I was notified that medications that directly affect the bloodstream may possibly affect me in retaining my eyelid extensions.


    I have read and discussed all the information with the technician, Stacey Velez, and have cleared all my doubts regarding the procedure for eyelash extensions.


    I authorize Stacey Velez to apply eyelash extensions on my natural eyelashes.
