The Diabetes Prevention Program has a proven track record for significantly reducing the risk of Type 2 Diabetes.This program is 26 sessions, spread out over 12 months. You will have a new family and support team for the next 12 months! Wellness 4 Prevention is seeking National Diabetes Prevention Program recognition. The information you provide with this questionnaire is HIPAA secure, confidential and deidentified when submitted back to the CDC through HabitNu.
