I hereby agree to read all the Policies and sign the policy checklist.  I will complete and sign all the relevant permission forms.  I agree that once this contract is signed, it becomes legally binding. 

I, the Childminder and my assistants, agree to the conditions as set out in this contract.  We agree to continually strive to provide the best care for your child and to keep you informed of your childs progress, development and daily activities.
