I, {parentguardianName}, hereby acknowledge and agree to the terms and conditions of this liability waiver as a condition of participating in the activities and using the facilities provided by Laugh N Play, LLC located at 4210 Ave U, Brooklyn, NY 11234 (hereinafter referred to as "Venue").
1. Assumption of Liability: I understand and acknowledge that participating in activities at the Venue involves inherent risks, including but not limited to physical injury, property damage, and other potential hazards. I voluntarily assume all risks associated with my presence at the Venue and the participation of my child(ren) in the activities.
2. Consumption of foods and beverages. I understand and agree that the consumption of any foods and beverages at the Venue is at my sole risk. The Venue has not provided any food or deleverage service to us and all said consumed foods and beverages are a sole responsibility of the host of the party and not of the Venue. Said foods and beverages were catered by a third-party catering service upon the request of the party host, and the Venue is not responsible for the handling of said foods and beverages. I agree to hold the Venue harmless and release the Venue for any claims or damages that may arise as the result of the consumption of the said foods and beverages at the Venue.
2. Supervision of Children: I agree and understand that I am solely responsible for supervising and monitoring my child(ren) during the entire duration of our visit to the Venue. I will ensure that my child(ren) will behave responsibly and follow all safety guidelines and instructions provided by the Venue's staff.
3. Photography and Social Media: I consent to the use of any photographs, videos, or recordings taken of me and my child(ren) during our visit to the Venue. I understand that Laugh N Play, LLC may use these images for promotional purposes on social media or other marketing materials without any further consent or compensation.
4. Use of Email Address: I authorize Laugh N Play, LLC to use my email address to send future promotional materials, special offers, and relevant information about the Venue. I understand that I may opt-out from receiving such communications at any time.
5. Release of Liability: In consideration of being permitted to participate in the activities and use the facilities at the Venue, I, on behalf of myself and my child(ren), hereby release, waive, discharge, and covenant not to sue Laugh N Play, LLC, its owners, employees, agents, and representatives from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or illness, including death, that may be sustained by myself, my child(ren), or any property while at the Venue.
6. Indemnification: I agree to indemnify and hold harmless Laugh N Play, LLC, its owners, employees, agents, and representatives from any and all liabilities, costs, expenses, or damages arising from any claims or actions brought by third parties resulting from my actions or the actions of my child(ren) while at the Venue. I have read and fully understand the terms of this liability waiver, and I voluntarily sign it with full knowledge of its significance and legal consequences.
