KHS America Lesson Squad Contest Media Consent & Release For Minors

I have read and fully agree to the terms of this waiver and release.  I understand and confirm that by signing this waiver and release I have given up considerable future legal rights that the above-named minor and I may have against the above-named Released Party.  I have signed this waiver freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me.  My signature is proof of my intention to execute a valid and unconditional waiver and release of all liability to the full extent of the law.  I am mentally competent to enter into such an agreement.

I HEREBY WAIVE AND RELEASE KHS America, Inc., (Released Party) and its subsidiaries from any and all liability to the above-named minor.  I am fully aware that my minor is involved in such above-mentioned activity, and on behalf of such minor, release from liability KHS America, Inc. and waive any and all claims such minor may have as a result of above-mentioned activity, and on any photographs or video submitted to KHS America, Inc. for use in future marketing, advertising, and educational materials.  I hereby waive any right that I may have to inspect and/or approve the finished product of the copy that may be used in connection therewith, wherein the above-named minor’s likeness appears, or the use to which it may be applied.  This waiver shall be binding on said minor, myself, his or her heirs, assigns and next of kin, and shall inure to the benefit of Released Party and its successors and assigns.

I declare that the foregoing is true and correct. I represent that the above-named minor is at least thirteen years of age and I am at least eighteen years of age at time of submission and am fully competent to sign this release.
