Consent, Disclaimers, & Financial Agreement

I, the undersigned owner or authorized agent of the owner, certify that I am over eighteen years of age and hereby consent to the behavioral assessment by Mindful Mentor Behavior Consulting, LLC. I understand that there is no guarantee in the treatment of behavior problems or disorders. I understand that some risk always exists with the ownership of and cohabitation with animals. I hereby assume any and all risk that would customarily and ordinarily occur by my participation in pet ownership, handling, training, etc. I hereby waive and release Mindful Mentor Behavior Consulting, its employees, owners, and agents of any and all claims while implementing recommended management and behavior modification techniques including specifically, but without limitation, any injury or damage resulting from the action of any animal, including my own.

I understand that an estimate of the consultation fee has been provided to me and that I am encouraged to discuss all fees attendant to such case before services are rendered. I agree to pay for the balance of all services at the time of services.

I understand that, as an organization without a veterinarian on staff, Mindful Mentor Behavior Consulting is unable to make medical evaluations, assessments, or diagnoses. Any medical information is provided as generic education and should not be taken as direct medical advice for my pet.

I grant permission to the Mindful Mentor Behavior Consulting, LLC to use my pet’s image (photographs and/or video) for use in publications to include, but are not limited to, educational material, client resources, general publications, social media, etc.

 

I understand that my name (first and last) will never be used in association with these images or videos, but that my pet's name may be used as an identifier. 

 

I hereby waive any right to inspect or approve the finished photographs or finished matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
