ASSR Owner Surrender Form

Please complete one form for each animal (or bonded pair) being surrendered. Understand that this form DOES NOT guarantee that we can accept your pet(s) into the rescue. We, like all rescues, are always full and can only accept a certain number of animals before we are out of space and time to care for everyone.

Priority is given to true emergencies and to those that are able to make a donation beyond the surrender fee to help cover costs of spay/neuter and the time that your animal(s) will be staying with us. Some animals end up staying for many months waiting for the right home to come along and they require food, bedding/litter, air conditioning, water, toys, etc during that time. The more willing you are to help us the better we are able to help you and your pet(s).

IF your pet(s) is accepted into the rescue we have a surrender fee of $50 per guinea pig and per already spayed/neutered rabbit with proof of spay/neuter from a verterinarian. The surrender fee for unaltered rabbits is $100 per rabbit. *If you cannot afford these fees you can still apply to surrender and we will still try to help if we have the extra funds available to do so.

Always Sunny Sanctuary and Rescue makes every attempt to find safe and loving homes for the adoptable pets surrendered to us. Please fill out the form entirely. If admitted, the detailed and accurate information you provide us makes it easier for us to place your pet in an appropriate home.

By signing this form, I state that I am the owner of the animal who is the subject of this Animal Surrender Form. To my knowledge, no other person has a right to this animal. I hereby surrender all rights to this animal if the animal is accepted into the rescue. I understand that once I relinquish this animal, the animal will not be available to be returned to me and I am no longer entitled to information about the animal. I have read and understand the terms of this Animal Surrender Form.
