Once this form is submitted, I will reach out via your preferred contact method within 2-5 days to choose a date for your appointment. I typically work Mon, Tues, Fri, and Sat. Once we settle on a date I will require a non-refundable $50 deposit that will go towards the price of the tattoo. Deposit must be send via Zelle, I accept cash or Zelle on the day of your appointment. Deposit is good for one reschedule if you let me know at least 48 hours in advance.
