CLUBS POLICIES
- Monthly payments reserve your child's spot in the club. If your child's payment is more than 10 days past due, the spot may be offered to someone else and membership is considered as cancelled.
  The monthly dues are automatically charged to the credit card we have on file. If the payment is declined, card details must be updated before the child attends the next club meet.
- If you miss a club meet or if the center is not open due to severe weather, we don't offer make-up meets, pro-rated dues, or refunds.
- For new sign-ups, club spots are not assigned until payment is received.
- You can switch clubs if there's a spot open by paying the switching fee. Switching may take up-to 14 days to process.
- To cancel a club, email us at least 14 days before your next monthly due date so we can stop payment.
- Membership prices are subject to change with 30-day notice.
- A parent or guardian must be present at the center during club meets.
- These policies may be changed from time to as needed with 30-day notice.

PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT

In consideration of services performed by Funtastik Labs, LLC., a Texas Limited Liability Company, who is owner and operator of Funtastik Labs, at Funtastik Labs today and for up-to one year from today's date, I hereby execute this Parental Consent, Release of Liability, and Indemnity Agreement (the “Agreement”) on behalf of myself and any children, relatives, or dependents identified herein (collectively ‘Participants’), and hereby agree as follows:

I and Participants are solely responsible for the supervision of my children at the Funtastik Labs facility and agree that Funtastik Labs, LLC. assumes no responsibility for their supervision. I further agree that it is my responsibility to review and comply with the safety guidelines and policies of the Company and assist with ensuring my children’s compliance.

I and Participants shall abide by all Funtastik Labs rules, instructions, and the directions of Funtastik Labs employees and representative. Failure to abide by those rules, instructions, and directions can lead to the immediate revocation of my rights to use Funtastik Labs services and facilities, without any right to refund or any payment.

I and Participants grant exclusive permission to Funtastik Labs to use my or Participants’ names, faces, likenesses, voices, artworks, activities, presentations, creations, or appearances in photographs or videos for educational and promotional purposes such as, but not limited to, brochures, flyers, website, and social media without compensation and without restrictions as to frequency and duration.

In the event of a medical emergency, I and Participants authorize Funtastik Labs, LLC. to transport my children to a hospital or emergency room for medical treatment and hold Funtastik Labs, LLC. harmless in the execution of such. I agree to be responsible for any associated transportation and medical expenses.

In consideration for being permitted access to Funtastik Labs facilities and activities, I and Participant voluntarily assume all risk of injury from participating in Funtastik Lab activities and agree to fully and forever waive, release and discharge Funtastik Labs, LLC., its affiliates, owners, members, investors, representatives, employees, instructors, contractors, landowners, landlords, affiliated companies or entities, successors, heirs, and assigns, of any liability or obligation that may arise out of or in connection with my activities or the activities of others while at Funtastik Labs.

By executing this agreement, I declare under penalty of perjury that I am doing so only for myself and/or Participants for whom I am authorized. If I or Participant or any legal representative files a claim for lawsuit out of my or Participant’s participation in Funtastik Labs, I agree to defend, indemnity and hold harmless Funtastik Labs, LLC.

I understand the terms of this PARENTAL CONSENT, RELEASE OF LIABILITY, AND INDEMNITY AGREEMENT and understand that this release of liability will prevent me and Participants from filing suit or making any claims for damages in the event of any loss or injury arising from participation in Funtastik Labs activities.
