This agreement is between Hush Connections LLC and First Name Last Name. The agreement is for the rental of equipment (including headphones, transmiters and proper hook-ups) for the above outlined silent event. The cost of the equipment rental is Rental Amount with a non-refundable deposit of Deposit Amount due by Date . The Deposit is required to secure your event and time.
