Thank you for signing up to be a part of Kiran’s Teen Workshop or Volunteer Opportunity for Teens! Individuals participating in a Kiran Workshop or Activity are required to read the following information carefully and fully understand before participating in any activity or program.
Hold Harmless Agreement:
WAIVER: I, the undersigned, hereby state as follows: (1) understand and acknowledge that I am or my minor child is about to engage in an activity which includes strenuous exercise and body contact that involves certain risks, as well as unanticipated risks which could result in injury or harm to myself, my child, my property, or third parties. I hereby expressly and voluntarily release Kiran, its officers, employees, volunteers, and instructors from all responsibility, liability, claims, demands, charges, duties, injuries, actions, suits, complaints and promises of any nature whatsoever relating to or deriving from mine or my minor child’s personal presence at the Workshop. Participation in the workshop is purely voluntary.

If my child has any food allergies, it is my responsibility to provide them with an alternative food option and I will not hold Kiran responsible for arranging the alternative food and beverage option.

Please note that some terms and conditions in this waiver may not be applicable to the specific activity covered.

Media Consent Form and Release for Minor Child
I am the parent/guardian of( Full name of child)*. I hereby grant Kiran and their agents the absolute right and permission to use photographic portraits, pictures, digital images or videotapes of my child, or in which my child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any Kiran publication or on the Kiran website.
CONSENT: We/I hereby certify that We/I are/am the parent(s) or guardian(s) of the above named child and do hereby give our/my consent without reservation to the foregoing on behalf of my child.NON-CONSENT: We/I hereby certify that We/I are/am the parent(s) or guardian(s) of the abovenamed child and do not hereby give our/my consent without reservation to the foregoing on behalf of my child.
