To be completed by the student's Financial Aid Office. Due April 1 (fall term and academic year) or October 15 (spring term).

A student from your institution is studying abroad with The Swedish Program and will attend an optional excursion to the Island of Gotland (fall term) and/or the Arctic Circle (spring term). There is an additional fee to attend the excursion, which includes round-trip transportation, accommodations, and most meals and activities. If the student's financial aid package does not cover this cost, the fee will be waived. Please confirm the information below in order for The Swedish Program to process the fee waiver.
