I, hereby acknowledge and agree to the following terms and conditions in consideration for my participation in the slime making workshop conducted by Once Upon A Slime:

 

1. Assumption of Risks: I understand that the slime making workshop involves the use of various materials and substances, and there may be inherent risks associated with these activities. I voluntarily assume all risks related to my participation, including but not limited to the risk of personal injury, property damage, or other harm that may arise during or as a result of my participation.

 

2. Release and Waiver of Liability: I hereby release, discharge. and hold harmless Once Upon A Slime, its employees, representatives, agents, and volunteers from any and all claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or to any property belonging to me, whether caused by the negligence of Once Upon A Slime or otherwise.

 

3. Photo and Video Release: I grant Once Upon A Slime, its representatives, and employees the irrevocable and unrestricted right to use and publish photographs and videos taken during the workshop for promotional, advertising, or other purposes, without compensation or notification.

 

4. Compliance with Instructions: I agree to comply with all instructions, rules, and guidelines provided by Once Upon A Slime during the workshop. I understand that failure to comply with these instructions may result in my removal from the workshop without a refund.

 

5. Age Requirement: I confirm that I am 18 years old or accompanied by a legal guardian to participate in the slime making workshop. If I am under the age of consent, my parent or legal guardian has reviewed and accepted this waiver on my behalf.

 

6. Severability: If any provision of this waiver is found to be unenforceable or invalid, such provision shall be severed from this agreement, and the remainder of the agreement shall continue to be binding and enforceable.

 

By signing below, I acknowledge that I have read this waiver and release of liability, understand its terms, and agree to be bound by it.
