Informal "Non-Enrollment" Withdrawal
*Use this option for any family who has decided not to attend for the 23-24 school year. For returning families, the withdrawal date will be 5/31/23. For new families, the withdrawal date will be the date they notified you. We do not have a signed master agreement on file OR their withdrawal date is within one week of their start date, and we are not claiming any attendance.
Formal Withdrawal
The student had a signed MA and was enrolled for at least a week. A work sample, parent grid log, report card, and AWRs are required.
Administrative Withdrawal - Non-Compliant
A non-compliant student who is being withdrawn by the school **ONLY CHOOSE WHEN DIRECTED BY LEADERSHIP**
Administrative Withdrawal - 5-Day Non-Residency Notification
Failure to provide an approved Proof of Residency document upon requested
Graduating Student
The student is graduating or earning a Certificate of Completion. If earning a CoC, only submit the survey if the student is NOT returning to the ATP program.
PLEASE DO NOT SUBMIT THIS SURVEY UNLESS YOU HAVE UPLOADED ALL NECESSARY DOCUMENTATION.
IF IT HAS BEEN 5 SCHOOL DAYS SINCE YOU WERE NOTIFIED OF THE WITHDRAWAL, PLEASE PROCEED AND THE STUDENT WILL BE MARKED ABSENT ACCORDINGLY.
