 PLEASE NOTE: This form is built to be completed on your mobile device. If you wish to submit additional photos of your unit, please upload them to a Google Drive folder and include your link in the appropriate field on this form.

I acknowledge that if this form hasn’t been returned within seven (7) days of my move-in date, it will be assumed that there is no previous damage to the property.

I acknowledge that Four Star will only accept the first check-in sheet submitted for my property unless my property has more than five (5) bedrooms. In this case, a check-in sheet will be accepted for every five (5) bedrooms. i.e., two sheets are acceptable for 6-10 bedrooms.

For Bedrooms in multi-level houses: please begin your numbering of bedrooms on the bottom-most floor of the home.

Optional: If you would like to submit additional general damages of your unit, please upload them to a shareable folder (Google Drive preferred) with a detailed description and location of each photo.
