By signing this application, I (the parent/gaurdian) understand that my family is responsible in supporting my child monetarily, physically, spiritually, and mentally. I promise to help them in raising the funds to the best of my ability.

I promise to help keep track of times and dates to ensure that my child is available for online meetings. I will do everything in my power to keep the Trip Leader appraised of any conflicts or issues. I understand that my child may be asked to give up their spot on the trip if we fail to meet the minimum requirements.

Finally, I understand that my participation and encouragement is vital to the overall group mentality. I agree to support my child and the missions group in this missions trip in everyway that I can.

Terms of Agreement
For the Student:

By signing below, I (the student) understand that I (and my parents) are responsible for my deposit, and inevitably the full cost of my trip.

I also understand that preparation for this trip is crucial, and promise to read the book given to me, and attend the online mission trip meetings. If I am unable to attend a meeting, I will let the Trip Leader know in advance. If I fail to meet these requirements, I understand that I may be asked to give my spot to another student who can comply with the requirements.

Finally, I agree to participate fully in the mission trip, regaurdless of what obstacles we may encounter. I understand that missions work is hard, and promise to give my whole self, allowing Christ to work through me. I understand that housing may not be what I am used to, that the enviroment may be challenging, food may be different or limited, and I fully welcome these opportunities.
