Instructions
1. This form should be only completed by directors or shareholders.
2. Before submitting this e-Form, you must use the Document Uploader at the end of this page to add and upload a pdf document which includes the ID Card / Passport and the Police Conduct certificate (issued by the Malta Police) for the responsible person and for each user.
3. Once you upload the pdf document and you click on the 'Submit' button, an authorised person will have to visit the Customs Excise Office at the address below to present the original copies of the Police conduct certificate for each user. The authorised person will need to present his/her ID card.

4. Once the the original copies of the Police conduct certificates are presented at the Customs Excise Office, the respective usernames and passwords will be remitted to the individual email addresses of the users.
Note: If the authorised person does not present the original copies within 10 working days from this electronic submission, this application will be discarded and you will need to start the process all over again.

The Customs Department will use the information provided to process this form in accordance with the applicable law. We may check information provided by you, or information about you provided by a third party, with other information held by us. We will not disclose information about you to anyone outside this Department unless permitted by law. The Commissioner for Revenue will treat your personal information in accordance with the General Data Protection Regulation 2016/679 to protect your privacy.
