    We are conducting a Community Health Assessment to better understand the health of the people who live in our county and the things that affect their well-being.

    Your participation in this survey will help shape the future of our county.

    • 1. Why is this survey important?
      Your answers to the survey questions are important to understanding the health needs and concerns of our community.
      2. What are these questions about?
      These questions cover individual and community life, physical and mental health, and well-being. Many questions have an option to not answer and all of them are anonymous: your answers cannot be linked to you personally.
      3. How will this information be used?
      Your input will help us create health programs and services that meet the specific needs of our community. It will also be used for reports, applying for grants, and informing our community of realities that exist.
      4. Who can take this survey?
      This survey is open to all who live in Calaveras County aged 14 and over.
      5. How long will this survey take?
      This survey will only take about 15-20 minutes.
      6. Why is my participation in this survey anonymous?
      We care about your privacy and want you to feel safe sharing your thoughts and experiences. Your answers will be combined with others and presented in a way that does not reveal your identity.
      7. Is my participation in this survey voluntary?
      Yes, your participation in this survey is voluntary. It will not affect any services or benefits you receive.
      8. How do I enter the raffle for a gift card?
      After answering the required questions, you will have the option to enter the raffle for a chance to win 1 of 50 gift cards valued at $45 each.
