Booklet mailout is optional. A copy of the booklet is viewable online within the course.
If you have a Calgary address, this form should be filled out at least 4 business days before the class start date or mailout will not be done.
If you have a non-Calgary address, this form should be filled out at least 6 business days before the class start date or mailout will not be done.
How it works:
1) Sign up and pay for the course (should already be done).
2) Fill in this form. Any form filled out with information which does not match the order number will be deleted, so please enter your information carefully and correctly.
3) Send an e-transfer to "ibstar (at) wealthsimple (dot) me". $19 for one booklet, $38 for two booklets, or $51 for three booklets. In the e-transfer "message", please type the last name of the student.
4) Submit the form.
There are no refunds for the mailout cost OR the cost of your course once the booklet has been mailed to you.
We will mail the booklets in excellent condition, usually the day after you fill out the form (We will not be mailing out booklets between December 15 and December 25). We are not responsible if there are delays with Canada Post or if the booklet arrives damaged. Thanks for your understanding.
