MEAL BREAK TIMES: LIST ALL TIMES BELOW. A meal break must be at least 30 mins long. A meal break must be logged for every 5 hours on shift, and completed prior to each 5th hour on shift for both on and off duty meal breaks. If you DID NOT take any meal breaks you must detail why below. Failure to log a meal break outside of bonafide emergencies may be grounds for termination. Shifts longer than 9 hours must have at least 2 meal breaks logged.
