BabyBeats Before Birth Informed Consent
Information you possess about your pregnancy, current health status or previous experience of unusual feelings with physical effort may affect the safety and value of your experience within a BabyBeats Before Birth class.
Your prompt reporting of feelings and/or effort during the class is of great importance. It is your responsibility to fully disclose information as requested. Your participation in a BabyBeats Before Birth class is voluntary. You are free to stop at any point.
During BabyBeats Before Birth, we aim to give you a calm and effective class. To achieve this, we ask that you never force any exercise, position or stretch. If you are unsure about any part of the class, please speak to your teacher. We ask that you never perform any move that you are not comfortable with. Please ask your teacher for an alternative if you do not feel confident or happy.
If you feel you are not well, it is best not to attend the BabyBeats Before Birth class. We do not recommend you attend a BabyBeats Before Birth class within 24 hours of any immunisation (e.g flu jab).
I have read the above safety guidelines and will ensure that I adhere to them.

I will seek the advice of my GP or Midwife before commencing BabyBeats Before Birth classes if have any concerns.
If any health issues, injuries or concerns occur within the block of classes, I will inform my teacher and seek medical advice were appropriate.
I acknowledge that in normal circumstances exercise should not harm my baby or in any way. I shall inform my BabyBeats Before Birth teacher of any medical or pregnancy related changes prior to commencing any class.
I give Beth Cunliffe, of BabyBeats Before Birth, permission to contact any of the contact numbers set out above in the event of an emergency.
