This Form is asking PERMISSION to hold a fundraiser not notification of so it is important that the person filling out the form understands the following...
- Form must be submitted AND approved before the fundraisermay begin.
- Invoices will not be paid until all money is collectedand fundraising/resale form iscompleted and returned to the business office.
- If the sponsor/coach does not follow through withthe procedures and protocols, including all forms filled out, the coach/sponsoris responsible for all expenses aspersonal expenses.
- It is the responsibility of the Sponsor/Coach to follow all fundraising guidelines set forth in policies 504.5 and 504.5R.
