  • Parent/Guardian Agreement to Supervise

    (Criminal Background/Fingerprinting Waiver)

  • I, acknowledge that my child, will be receiving services by as a contractor of River's Edge Academy Charter School from   Pick a Date   to   Pick a Date   

  • I understand that contractors for public charter schools are required by law to complete a state and federal criminal records check with fingerprinting by the Oregon Department of Education if the contractor has direct, unsupervised contact with a student.

     

    I agree to directly supervise my child or children when my child or children are receiving services from Contractor to meet the requirement of law. I understand that my supervision means that Contractor has not been fingerprinted and has not had a state and federal criminal records check.

     

    I agree to waive all claims against River's Edge Academy Charter School for any action or inaction taken by Contractor connected to my child or children and these services. I will be responsible for any failure to supervise my child or children.

     

    I will inform River's Edge Academy Charter School if I am no longer supervising my child or children during Contractor's services.
