Waiver: By signing below, I am aware that any physical sport activity may cause accidental injury or harm among the athletes, and I assume any and all possible risk that may cause injury, illness, or death arising to such activity. I agree to waive my right to pursue any claim against the Picayune School District or Swingking.
Media Release: SwingKing University has my permission to use the player’s photographs or videos publicly to promote the academy. I understand that the images or video may be used in print publications, online publications, presentations, websites, and social media. I also understand that no royalty, fee or other compensation shall become payable to me by reason of such use.
Travel Team Agreement: I have read the email listing the terms, benefits, and costs of the Swing Kings travel team. I agree to pay the deposit of $400 by August 15, 2025 and the remaining balance by paying $85 on the 15th of every month beginning in September and ending in June (10months) to reserve my spot on the team and the elite training, uniforms, gear, tournament fees, benefits, etc., as per the email.
