I. TERMS AND CONDITIONS: The borrower is personally responsible for care, repair, loss or theft of the below described musical instrument while in his/her care. The student must immediately report any damage or loss of said instrument to the music instructor and repairs may be at the borrower's expense. No person other than the borrower will be permitted to use the instrument and it must be returned to the school in its original condition at the end of the loan period. Near the end of the school year, the instrument(s) will be checked over by the music instructor or a competent repair technician.
In the event of damage (beyond normal wear and tear) that occurs with an instrument you are required to fill out our Damage Repair Form within 14 days of damage.
II. LOAN PERIODS: Normal loan(s) shall terminate at the end of the current school year and will not be extended unless a separate ''summer loan" is approved, based on approval with the instructor and filling out our programs Summer Loan Form.
III. MAINTENANCE:
To offset the expense of routine instrument maintenance, repairs, and replacement we request a nonrefundable voluntary contribution as described below:
SUGGESTED CONTRIBUTION PER INSTRUMENT:
Please note that our current requested donation amount is based on the repair cost and upkeep cost of the instruments. Our aim is to make the program financially sustainable. All donations are voluntary and will be treated with confidentiality.
IV. Use Intent:
Any school instrument that leaves the school premises needs a loan agreement.
Any shared use of a school instrument at the school does not need a loan agreement, as long as the usage is confirmed with the music teacher. All basses and cellos used on school premises are considered shared instruments and therefore do not require to have a loan agreement filled out.
Any use of an additional dedicated individual instrument at school requires a signed loan agreement. For example, if a student has one instrument at home and uses a dedicated second individual not shared instrument at school, they will fill out this loan agreement for two instruments and we request a contribution for two instruments to cover the costs for repairs as mentioned above.
If a student first signs one loan agreement and later decides to loan an individual instrument in addition, they are required to sign another loan agreement for that instrument only.
