The form was prepared by the Archdiocese of Miami and is processed by the Office of the Chancellor for Canonical Affairs. This is a request for granting the archdiocesan faculties to the visiting priests and deacons (not lay speakers) for a specific period of time or events.
Who Fills this Form:
This request is made by a Pastor or a Director of the Archdiocese of Miami who asks for the faculties on behalf of the visiting clergy.
Certificate of Aptitude / Letter of Good Standing:
At the end of the form you will have an option to upload an electronic version of the ADOM Certificate of Aptitude for this visiting priest or deacon. The certificate has to be filled out, signed and sealed by the Bishop or Superior of the visiting clergy. Only ADOM approved Certificate of Aptitude will be accepted (see below). The faculties will not be granted until both: the form and the certificate are submitted to the office. Therefore, it is ideal to have the Certificate ready for upload when filling out the form. However, you may mail or email it separately, or ask that the Ordinary would do so directly to Chancellor for Canonical Affairs.

Financial Considerations:
This request for faculties might be connected with monetary renumeration for the services rendered (fees, salaries or stipends). Please follow the government and archdiocesan rules in this regard. Caution: a cleric who has a US travel visa or US student visa IS NOT ELIGIBLE for any payment whatsoever. The religious who work for their communities should be remunerated only by sending a proper payment to their religious superiors and communities. All others who are eligible for payment MUST provide to the responsible entity (parish, school, organization, etc.) filled IRS W-9 Form prior to the engagement.
IMPORTANT: the form and the Certificate of Aptitude must be received by the Chancery Office at least 14 days prior to requested time frame or event.
