  • Our goal is for your dog to have a comfortable grooming experience. We go through each step with gentle encouragement, if your dog shows stress we pause. During the grooming procedure, everyone's safety comes first, both the people and the animals.

    This form provides details on our grooming process and helps us get to know your pet.

  • Vaccines

    It is strongly advised that all pets be up to date on all vaccinations, and Rabies immunization is required to get grooming services.
  • Ticks & Fleas

    We make every effort to be a flea-free environment. If your pet has fleas, an additional $10-$20 will be added for a flea bath (depending on the size of your dog). You can reschedule your appointment after the matter has been resolved if you do not want them to get one.
  • Matted Coats 

    Matted coats can cause a variety of skin and health problems. Matted fur does not allow air circulation to the skin, causing hot spots, bacterial and fungal infections. Fleas, tickets, maggots, and other parasites may lurk in the coat causing further skin infections. Matted fur also pulls and binds, causing pain to your pet when they move or lay on mats. The inability to dry properly leaves the skin underneath raw and inflamed. Removing a densely matted coat requires shaving to the skin, revealing the underlying skin irritation and sores. The process could result in nicks, cuts, or abrasions. Itching, skin redness, self-inflicted irritations or abrasions, and hair failure are all possible side effects of mat removal techniques. Grooming visits should be scheduled every 4-8 weeks for the best defense. By checking the box here, you realize that your pet may need to be shaved to remove matting and that you understand the risks involved.
  • Accidents

    Although accidents are extremely rare when working with animals, they do occur. Grooming equipment is sharp, and while we employ extra caution and care in all cases, cuts, nicks, scratches, quickening of nails, and other issues are conceivable. This is most likely to happen when an animal is wiggling or moving around. The safety and comfort of your pet is our priority. If an accident happens, you will be alerted. If the situation is critical and the owner is unavailable, Cha Cha's Dog Daycare will seek immediate medical care for your pet at the nearest veterinarian clinic.
  • Authorization for Emergency Veterinary Care

    In a medical emergency while in our care, this release grants ChaCha's Dog Daycare full authority to seek medical attention from your family vet (listed below) or the nearest vet clinic.

  • Cancellations/No Shows

    An automated text is sent to remind you of your appointment (please confirm that we have the correct number on file). If you will be late for your appointment, please let us know as soon as possible. All cancellations or reschedules require at least a 24-hour notice to provide us time to fill the spot with a customer from our waiting list. Grooming appointments will be held for 15 minutes after the stated drop-off time. Appointments more than 15 minutes late may be canceled due to high demand. A $20 fee will be charged for no-shows and appointments canceled with less than 24 hours notice.
  • Satisfaction

    Your satisfaction is extremely important to us. If you are dissatisfied with your groom for any reason, please get in touch with us within 48 hours, and we will organize a time for you to return for a free adjustment at the next available time.
  • Pick Up

    We have limited space; please pick up within 30 minutes of grooming completion to keep all pets comfortable. After 30 minutes, if your pet is still with us, a $1.00 per minute charge will be applied.
  • Photographs

    By signing this release form, you permit us to photograph your pet for use in customer files and for company social media.
