Bus will DEPART Friday, August 2 @ 8am! Bus will hopefully leave @ 5pm Sunday, August 4 from Wascana Canoe Club, that means arrival into Calgary Canoe Club @ 12am-1am, a text or call from your athlete on when arrival will be.
The athlete will:*

Permission Form and Medical
We, Parent/Guardian (full name)* (parent/guardian initial, over 18+) hereby give permission for our son/daughter; to travel from Calgary, AB to Regina, SK from August 2-4, 2024.

Our son/daughter is attending the Prairie Divisional Championships with the Calgary Canoe Club. Coaches and/or chaperones throughout the trip will supervise athletes.

I give my permission for medical assistance to be administered should they become ill or involved in an accident initial* (parent/guardian initial, over 18+).
CCC has collected information when you join the program but did not collect Health Card Numbers at that time. Please provide the athlete health card number before the trip (just in case we need a trip to the hospital).*
