    IAB FIELD EMERGENCY PLAN

    Use this form to describe your emergency plan for field work off of the UAF campus. This plan should help you and emergency responders maintain communication, locate field personnel, and negotiate an emergency if one arises.

    INSTRUCTIONS:

    1. IMPORTANT: Regularly save your work by clicking the "save and continue later" button at the end of each section. You will receive a link to the document you saved. Unsaved progress will be lost.

    2. Navigate using the tabs at the top of the form or the buttons at the bottom of each section. 

    3. The level of risk and remoteness of your trip will determine the amount of detail required. Skip questions that do not apply to your trip. You may upload existing emergency plans, documents, and files throughout the form to avoid redundant efforts. 

    4. Submit your plan to IAB by clicking the submit button at the bottom of the last section. The Safety Office will provide you with a PDF of your plan after review and approval by:

    1. Principal investigator or team leader if the form was not submitted by them. Be sure the PI or team leader has approved the submitted version of this plan.
    2. IAB safety officer
    3. IAB director
    4. Environmental Health Safety and Risk Management (EHSRM) if your activity includes minors, the use of firearms or explosives, non-conventional modes of travel (this includes most forms of transportation other than on-road vehicles or commercial travel), or if it is requested by a reviewer. 

    Start this process early and please submit this plan at least two weeks before departure. Submit this plan at least four weeks before departure if your trip involves the use of firearms or explosives, ATV's, snow machines, boats, or the participation of minors. The approval process and trainings, particularly for remote travel and activities involving minors or unconventional modes of travel, may take several weeks to complete.

  • Section II: PARTICIPANTS

    List all known participants and provide an emergency contact for each. Inform IAB of any changes. You can update this list as plans evolve.

    Describe the location of field sites as accurately as possible. You may use the expandable table, provide other descriptions, or attach files and maps as appropriate. Include GPS coordinates when available.

    Use this section to provide information that may help you quickly respond to an emergency and help emergency services find you and understand your needs if they are called to respond.
