    By completing this form, I am submitting an appointment request and price quote inquiry. In response, I will receive an email with service package option pricing that Nikky Atkinson decides that will be the best option to achieve my hair goals with the current state/condition my hair is in. The package pricing sent are baseline prices for the services, and could be adjusted upon further consultation. I will also receive current appointment date and time options available that would coordinate with the time/date stated above. I understand that once a date is selected, I will receive an invoice for a $50 deposit to hold the spot for my appointment. This $50 deposit covers the consultation and is non refundable. I understand that this deposit will be applied to my appointment and taken off the service cost the day of my appointment. I also understand that the date of my appointment will not be held for me until this appointment deposit is received by Nikky Atkinson Hair & Makeup. I understand Nikky Atkinson's strict 48 hour cancellation policy, and if I do cancel my appointment with less than 48 hours notice, I may be subject to a late cancellation fee of up to 50% of my booked service.
