This form is an in-depth consultation that helps me ensure that we are booked for the proper amount of time needed to give you your desired look, all while respecting your beauty budget. I understand that this is a general consultation intake form designed so that Dean at Simply Colour can help me choose which New Guest Service option I'm best suited for.If I desire a more thorough explanation of the coloring process or a detailed analysis of my current hair, I will need to schedule an in-person consultation.I understand that submitting this form does not guarantee that my hair goals can be achieved in one session. Salon Policies
Cancellation Policy - Your appointment is very important. I understand that sometimes schedule adjustments are necessary. Therefore, I request at least 72 hours' notice before your scheduled appointment time for cancellations or rescheduling of appointments. Any appointments canceled, rescheduled, or changed without 72-hour notice will result in a charge equal to 100% of the reserved service amount. Redo Policy - I strive to offer my clients the highest level of customer satisfaction. If you need help with your cut or color, let me know within 7 days of your visit, and I will be happy to correct the issue within the limitations discussed during your consultation, as there are NO REFUNDS on services. Should you make a retail purchase that is damaged or malfunctioning, I will accept retail exchanges for the same product for 14 days from your initial purchase. Late Arrival Policy - All appointments begin and end on time to maintain the salon schedule. If the treatment does not start on time due to client tardiness, the treatment time will be reduced accordingly, and you will still be required to pay the total price. If a guest is more than 15 minutes late, the appointment may need to be rescheduled and charged as a cancelation. Please notify me of any late arrivals via text.*

Child Policy - Please arrange all necessary child care before your visit. I also ask that you attend your appointment alone. My appointments are one-on-one, and I have limited space in the studio.*
