    Please fill out the information below to register for classes. Please only use this form if you have already filled out the main registration form for your first student.

  • CLASS REGISTRATION

    Please select the classes you want to register for below. You may select multiple classes on each day if needed.

  • Please pay the registration fee by the first day of class. Cash or Check accepted.

    Registration Fee is $20.00 for one child and $25.00 for more than one child in the immediate family.  Registration Fee is non-refundable.

  • DANCE PROGRAM AGREEMENT

    In consideration of the participation of my child in the Dance Program at The Dance Factory, I hereby agree to pay all fees and dues for my child’s dance according to the class enrollment described in the Registration Form.
